Mt. Airy Day 2022

We are excited that Mt. Airy Day is back this year! June 11th 12-7 pm.  We are celebrating 50 years!

 

Please complete this form and submit with the necessary proof of insurances and health certificates, if applicable, by Jun 3, 2022.

LOCATION: Germantown Ave. Between Upsal and Johnson Street.
COST:

  • 10×10 space fee: $75.
  • Table rental fee:$25 (Please note that we are encouraging Vendors to bring their own tables as we may be short of volunteers to set up and break down)
  • Food vendor : $300

::ATTENTION TO ALL FOOD VENDORS::

THE CITY OF PHILADELPHIA DEPARTMENT OF LICENSE AND INSPECTIONS REQUIRES ALL VENDORS TO HAVE:

1) Annual Business Privilege License $50.00 (plus processing fees)

2) A Philadelphia Tax ID Number (NO COST- Must register online)

3) Health License 125.00 looks like it is now $150? (PRICES MAY CHANGE PLEASE CALL THE NUMBER BELOW TO VERIFY) PHILADELPHIA DEPT. OF LICENSES AND INSPECTIONS 215-686-2400

Your application process will be completed once payment is received. By submitting your application, you agree that you have read this application in it’s entirety and agree to follow Mt. Airy Day Vendors rules and guidelines.
